Welcome to DI 2010!

Congratulations on your acceptance into Discovery Internships! We are excited that you have chosen to spend your summer with us. This is certain to be an inredible experience: Fun, but educational. Challenging, but rewarding.

Please reference the location specific pages (right column) for activity calendars, directions, packing list, and other important information. You will find answers to many questions in the DI Welcome Letter for each specific program.


Forms

There are several required forms that must to be submitted prior to the program start date. Click here to download and complete these forms. If you have been accepted into the program and this link is not currently active for you, please email Aviva Vikstrom, our Director of Operations, for assistance: aviva@careerexplorations.com. If you need help submitting your forms, please visit our help page for instructions and assistance.

Medical Form: A physical examination by a licensed physician after June 1, 2009 is required for all participants. Health insurance is mandatory for participation in Discovery Internships, and proof of insurance is required. The medical form and physician’s report are due by May 1, 2010.

Summer Contact Information: We require detailed contact information for your parents/guardian as well as emergency contact information. This form is due prior to May 1st.

Residential Students Only:

Transportation Form: DI requires the details of your travel information to be submitted by June 1st.

Visitor Consent Form: In order to spend free time with family or friends in New York City, you must have permission from a parent/guardian on file in the DI office. The Off-Campus Consent Form must be submitted online by May 1st.

Credit Card Deposit Form: DI requires program participants to have a credit card on file to cover unplanned expenses such as lost room keys, damaged property, or medical expenses. This form is due prior to May 1st.

2010 Intern Handbook: This manual contains important information to help you get the most out of your Career Exploration experience and carefully details the rules and regulations at Discovery Internships. All interns and their parents are requested to read the handbook prior to the start of the program, and are expected to understand and abide by the rules at all times.
