If you are having any technical difficulties using the electronic forms provided, please make sure you have Adobe Reader 9.0 installed on your computer. Adober Reader is free software associated with the popular PDF file format. You can download Adobe Reader 9.0 or upgrade your version of Reader here. If you have an out of date version of Reader, you will not be able to submit the electronic forms. It should only take a few minutes to download the appropriate software and will save you time in completing these form, save us time in processing forms, and save the unnecessary use of paper.

Once you have Adobe Reader 9.0 installed on your computer, you should be able to download and complete all required paperwork. To return the forms to DI, all you need to do is press the "submit" buttom in the upper right hand corner of each form once you have entered the required information. You will be asked if you are using a desktop email client (Outlook, Entourage, Thunderbord, etc) or an internet email client (if you access your email through an internet browser such as Internet Explorer or Firefox). Select the appropriate method and follow the prompt to send a copy of the completed form to us via email. We appreciate your cooperation and timely response in this regard. If you were able to successfully send the email, we received your form.

If you are having difficulty downloading Adobe 9.0 or submitting these forms for other reasons, you can save the form, fill in the appropriate information, and then print and fax the form back to us. Please fax completed forms to 917-210-3912.

If you have questions about your internship placement, please contact your Program Director.

If you have been placed into an internship, do not contact your internship provider directly. If you are awaiting placement, please keep in mind that placement can take 8 weeks or longer from the time of acceptance. Our Program Directors will keep you informed of any developments concerning your placement.
